Important Notice Regarding Dependent Enrollment and Family Status Changes

Employees who enroll eligible dependent family members  (including, but not limited to, domestic partners) in University benefits are required to notify Human Resources within 30 days of any change in family status relevant to such individuals’ eligibility for continued coverage under University benefits, including (but not limited to): marriage, divorce, legal separation, birth or adoption of a child, death of a dependent, loss of dependent eligibility, or change in spouse or domestic partner’s employment status and associated eligibility for alternate coverage.

Failure to provide timely notification of such family status changes may result in disciplinary action and could require the employee to reimburse the University for any ineligible expenses incurred during the period of non-compliance. Moreover, by enrolling in coverage under university benefits and signing this Employee Handbook, employees (without limiting the University’s rights to seek recovery of such expenses through litigation) grant consent to the University to recover ineligible expenses from their wages and/or vacation time. It is the employee’s responsibility to ensure that all enrolled dependents remain eligible for coverage and to promptly report any changes that affect eligibility.
